Membership valid for 12 months from date of purchase. Full payment is due at enrollment. Auto-renews annually unless canceled in writing. No refunds for unused services or mid-year cancellations. Benefits apply only at Georgia Dental Wellness and do not cover lab fees, retail products, or specialist referrals. Discounted services must be completed during the active membership period.
